Maricopa Health Plan is NOT currently accepting letters of interest for the following specialties:

NC = not a covered service
**Please note: The decision to add a provider to the network is based on current network adequacy in
the provider’s service area. Although a specialty is not listed on the above grid, our network may
already be adequate in your area.
If you are interested in becoming a contracted provider with our Plan, simply fill out the
Letter of Interest Provider Form
and fax to our Network Development Department.
